Social Worker/Mental Health Counselor - Part-time

 

We are looking for a dynamic practitioner with a passion for recovery and an unwavering belief in people’s ability to heal to join our cohesive and dedicated clinical team at Gould Farm as a licensed clinical Social Worker or Mental Health Counselor.  Our program integrates psychiatric rehabilitation and trauma-informed clinical therapy within an open setting. Supervised by the Clinical Director and working as part of a multidisciplinary collaborative team, the Social Worker will work with program participants in our Transition program in the Berkshires to provide a continuum of coaching, counseling, traditional therapy, and care coordination. The Social Worker/Counselor develops person-centered service plans and documents treatment activities that support Gould Farm’s recovery model and guest treatment. 

 

Primary Responsibilities include, but are not limited to: 

 

  • Ability to work independently as well as part of a multidisciplinary team;
  • Work with approximately 6-8 program participants, providing a continuum of coaching, counseling, and traditional therapy;  
  • Work with program participants to develop service plans and support recovery goals; maintain accurate records, ongoing progress reports, and discharge summaries to depict a narrative of guest activities, abilities, and development in the program;
  • Work as a member of the Clinical Team in conjunction with the attending psychiatrist to ensure continuity of treatment for individuals in the program;
  • Familiarity with other services and resources within the community and working closely with them to provide information, referrals, advocacy, and support to participants when needed;
  • Be available to intervene and respond to crises;
  • Work closely with the families of program participants to ensure all voices are included in a network of care;
  • Work with the House Advisors and Transition Counselor to support a resident in maintaining 30 hours of structured work and activities;
  • Actively participate in cultivating a nurturing community culture;
  • Participate in team meetings, including weekly doctors' meetings and Transition Program team meetings.

 

Qualifications:  Master’s Degree in social work or related field and 2-3 years experience working in behavioral health with major mental health challenges.  LICSW or LMHC (or eligibility) preferred. Understanding psychiatric rehabilitation and recovery is a plus. Maintain professional licensure during employment. Part-time 10-12 hours per week.

 

Compensation: Starts at $30.00 per hour,
