Job Description

Social Worker at Gould Farm’s Boston Area Program, Medford, MA

We are looking for a dynamic practitioner with a passion for recovery and an unwavering belief in people’s ability to heal to join our dedicated team at Gould Farm’s Boston Area Program.  As part of Gould Farm’s full continuum of services, we support adult guests (our descriptor for residents) with mental health challenges like depression, bipolar disorder, schizoaffective disorder and schizophrenia transition from our supportive psychiatric rehabilitation community made up of residents, staff and their families, located on a working farm in the Berkshire woods, to begin to rebuild and regain their lives at our Fellside House in Medford.

Gould Farm’s transitional residential program at Fellside in Medford serves as a launchpad for guests as we assist them in setting up their next chapter post-Farm, including attaining meaningful work, clinical services, and recreational and wellness-centered activities. As part of a tiered support approach, Fellside’s Extended Community program provides ongoing clinical case management to guests throughout their transition.

Supervised by the Clinical/Program Director, and working as part of a collaborative team, the Social Worker will work with individuals and groups providing a continuum of vocational/educational coaching, and clinical case management support to facilitate smooth transitions for guests from the Farm to Fellside, and from Fellside to independent living.

Responsibilities will include:

  • Working independently as well as part of a collaborative team to conduct assessments, explore vocational, educational, and wellness-oriented opportunities and resources, person-centered goal setting, (including the development of an individualized structured plan for meaningful work), individual and group skills counseling, and discharge planning.
  • Cultivating relationships with area resources to connect guests to local opportunities.
  • Working closely with family partners and community providers/collaborators to ensure all applicable voices are included in a network of care to promote guest well-being.
  • On-call rotation is a required part of the schedule.

As a Fellside staff member, this role includes living out the values and spirit of Gould Farm through encompassing a strengths-based perspective towards recovery and actively participating in the ongoing nurturing of a Gould Farm Fellside-based community in the Boston area.
