RVCC is looking to hire a Case Manager for their SOAR Program!

Overview of Job Responsibilities:

  • To work as an early intervention specialist with clients who have been identified at risk for substance-use disorders
  • To provide comprehensive care coordination of services for clients at an assigned location
  • To coordinate treatment goals
  • To assess their clients’ overall mental status
  • To help clients set and work towards long-term goals
  • To connect clients and their families with the resources they need to successfully manage their conditions on their own and engage parents
  • To transport, as needed, clients to community events and services
  • To facilitate weekly psycho-educational groups
  • To oversee & plan after school & summer youth development program/activities
  • To coordinate services with the mentors, school, families to meet the goals of each ISP
  • To provide or refer clients to tutoring and homework assistance
  • To obtain required documentation from clients. Complete initial Acuity Assessment with timely Acuity Re-assessments.
  • To provide or ensure that English/Spanish written and/or verbal interpretation for Spanish-speaking clients of all ages and their families/significant others is provided when necessary. Arrange interpretation for other languages as needed

 

Job Requirements:

Education Level - Bachelor’s Degree in counseling, psychology, social work or related filled

Years of Experience - Prefer 1-year experience working with substance-use populations.

Licensing - Valid MA driver’s license

Skills/Abilities - Bilingual (English/Spanish) speaking abilities preferred. Excellent verbal and written communication skills; interpersonal skills; crisis intervention skills; organizational skills; attention to detail; computer skills; good customer relations skills; knowledge of legal and ethical issues pertaining to the profession.
