This role provides intensive case management to youth identified to be at risk of using substances.
Essential Job Functions:
- Adheres to all applicable state and federal regulations; ensures compliance with the organization’s policies and procedures, code of conduct, and all applicable regulatory requirements.
- Provides intensive case management, including regular meetings with caregivers, school personnel, law enforcement, and other stakeholders.
- Performs intake interviews, needs assessments, and individual service planning.
- Maintains required documentation in timely manner as outlined in Brien policy.
- Develops and coordinates recreational activities for youth.
- Refers families to additional needed services.
- Provides Wraparound services.
- Prepares and submits monthly data and activity reports.
- Participates in all relevant and/or required meetings, trainings, and supervisions.
- Conducts community outreach events.
- Meets with youth on a regular basis to work on skills teaching, included but not limited to self-regulation and executive functioning skills.
- Conducts home visits, as appropriate.
- Drives youth to and from outreach activities.
- Provides tutoring and/or other academic supports.
- Encourages youth engagement in services to maintain expected enrollment numbers dictated by the contract.
- Prepares and participates in coalition groups to educate and inform youth, families, and community members.
Other Duties:
- Performs additional duties, as directed by management.
- Participate in on call rotation.Qualifications, Experience, and Education
- Bachelor’s degree in counseling, psychology, social work, or related field.
- Master’s degree in counseling, psychology, social work, or related field preferred.
- One year of experience working directly with youth, families, and substance use issues.
- Demonstrated knowledge about the community served and available resources.
- Demonstrated oral and written communication skills.
- Ability to work with culturally diverse populations.
- Ability to work school hours and non-traditional hours as needed to accommodate youth and families.
- Ability to use electronic medical record system, spreadsheets, email, and other computer applications.
- Valid Driver’s License and use of personally insured vehicle for agency business.
Qualifications, Experience and Education:
