This role provides intensive case management to youth identified to be at risk of using substances.

 

Essential Job Functions:

  • Adheres to all applicable state and federal regulations; ensures compliance with the organization’s policies and procedures, code of conduct, and all applicable regulatory requirements.
  • Provides intensive case management, including regular meetings with caregivers, school personnel, law enforcement, and other stakeholders.
  • Performs intake interviews, needs assessments, and individual service planning.
  • Maintains required documentation in timely manner as outlined in Brien policy.
  • Develops and coordinates recreational activities for youth.
  • Refers families to additional needed services.
  • Provides Wraparound services.
  • Prepares and submits monthly data and activity reports.
  • Participates in all relevant and/or required meetings, trainings, and supervisions.
  • Conducts community outreach events.
  • Meets with youth on a regular basis to work on skills teaching, included but not limited to self-regulation and executive functioning skills.
  • Conducts home visits, as appropriate.
  • Drives youth to and from outreach activities.
  • Provides tutoring and/or other academic supports.
  • Encourages youth engagement in services to maintain expected enrollment numbers dictated by the contract.
  • Prepares and participates in coalition groups to educate and inform youth, families, and community members.

Other Duties:

  • Performs additional duties, as directed by management.
  • Participate in on call rotation.Qualifications, Experience, and Education
  • Bachelor’s degree in counseling, psychology, social work, or related field.
  • Master’s degree in counseling, psychology, social work, or related field preferred.
  • One year of experience working directly with youth, families, and substance use issues.
  • Demonstrated knowledge about the community served and available resources.
  • Demonstrated oral and written communication skills.
  • Ability to work with culturally diverse populations.
  • Ability to work school hours and non-traditional hours as needed to accommodate youth and families.
  • Ability to use electronic medical record system, spreadsheets, email, and other computer applications.
  • Valid Driver’s License and use of personally insured vehicle for agency business.

Qualifications, Experience and Education:

 

  • Bachelor’s degree in counseling, psychology, social work, or related field.
  • Master’s degree in counseling, psychology, social work, or related field preferred.
  • One year of experience working directly with youth, families, and substance use issues.
  • Demonstrated knowledge about the community served and available resources.
  • Demonstrated oral and written communication skills.
