Description

  1. Assess the counseling needs of the clients.

  2. Assess clients using the ASI or other standardized assessment tool or using other evidence-based validated assessment tool and diagnosing clients for substance disorders using the DSM-V.

  3. Determining the appropriate level of care based on ASAM.

  4. Obtaining previous records that are relevant to the current treatment episode.

  5. Collaborating with the client in order to develop a written treatment plan that is client-centered and recovery oriented and includes goals and measurable objectives.

  6. Providing the substance abuse counseling services specified in the client treatment plan.

  7. Reviewing clients throughout the treatment episode according to ASAM, to determine the need for continued services or discharge/transfer.

  8. Reviewing and, where necessary, revising the substance abuse counseling portion of the client treatment plan to address emerging problems.

  9. Developing the client discharge/transfer plans to ensure movement to the appropriate levels of care.

  10. Contacting referral sources, providing case consultation and coordination with referral sources (for example: mental health treatment providers, criminal justice agencies, schools, employers.

  11. Participating as a member of a multidisciplinary team for assigned clients.

  12. Providing active case consultation; and

  13. Documenting all counseling and education services, assessments, reassessments, referrals, and follow-up in the client's clinical record, providing appropriate signatures and dating of such entries, including those made in electronic records.

  14. Attends daily and weekly staffing meetings per schedule.

  15. Recognizes, responds to and assists in psychiatric crisis situations and provide the intervention necessary.

  16. Adheres to all agency policies and procedures.

  17. Performs other duties assigned.

  18. Being sensitive to Cultural Issues of a patient based on their job position 

  19. Reporting Suicidality/Homicide of a patient based to their job position.

  20. Ability of competency and understanding to Addition based on their job position

  21. Reporting and responding to issues of Trauma of a patient based on their job position

  22. Reporting and responding to issues of Dual – Diagnosis issues of a patient based on their job position

  23. Adherence to all mandatory reporting requirements for abuse and neglect.

  24. Adherence to Code of Ethics.

 Position Requirements

A minimum of 2 years’ experience in chemical dependency and psychiatric setting preferred

Graduate from an accredited college or university with a minimum of master’s degree in Psychology, Social Work, Counseling, or health-related field.

Knowledge of Joint Commission requirements preferred.

CPR certification required.
