Case Manager- Full Time

Responsible for providing coaching and advocacy by supporting the resident’s

progression through educational/vocational growth which will support job readiness and self-efficiency. This position is also responsible to assist the resident back into the community by identifying and solidifying supportive resources.

Primary Services:

• Assess the resident’s needs in regards to vocational or educational services.
• Offer residents’ coaching with resume writing, job searching, educational opportunities,
• budgeting and aftercare.
• Complete accurate documentation in a timely fashion in accordance to licensing
• requirements and Foundation policies and procedures.
• Identify and connect residents to community resources which will support growth and independence.
• Facilitate a psychoeducational group.
• Establish, co-ordinate, and transfer residents for necessary appointments.
• Provide aftercare follow-up calls and support while maintaining documentation.
• Complete ESM forms for each assigned resident.

Additional Responsibilities:

• Provide crisis intervention and ensure the safety of residents.
• Attend meetings with care providers as needed.
• Participate in regular supervision, staff meetings, training seminars and case review.
• Adhere to all policies and procedures.
• Assists residents with daily activities of living and residential policies and procedures.
• Assist in the supervision of housekeeping and maintenance of the residence.
• Maintain current CPR, First Aid, and annual TB screening.
• Performs other duties as assigned.

Qualifications:

A Bachelor’s degree is preferred OR some combination of schooling in the related field with a minimum of a year experience in addiction treatment or in a related field with supervision. Also must maintain a working driver’s license.

Job Type: Full-time

Salary: $20.00 - $23.00 per hour
