We work alongside individuals with developmental and mental challenges who experience addictions recovery, trauma, crisis, and day-to-day situations.
The Associate Care Coordinator (ACC) will assist a team of Care Coordinators working at Affiliated Partner and/or Material Subcontractor agencies in their respective region. The ACC will support Care Coordinators in providing coordinated Long Term Services and Supports (LTSS) care management services as an integrated member of care management team to youth and adults in need of services.
Assist and provide administrative support to the LTSS Care Coordinator in the performance of service delivery Enrollee.
Maintain regular phone contact with Enrollee to monitor and coordinate LTSS Care Plan as directed by the Care Coordinator.
Work with the Care Coordinator to update periodically the LTSS Care Plan to reflect the Assigned or
Enrollee’s changing needs.
Complete all required documentation in a timely manner.
High School Diploma and two years of relevant experience. Previous experience with individuals who hold complex LTSS needs and bilingual communication skills are a strong plus!
Ability to use Care Management Software to document and coordinate services.
Strong interpersonal skills regarding developing a working relationship with a variety of individuals in a variety of context
Strong organizational skills with attention to detail, multi-tasking and prioritization skills, and ability to thrive in a team-based environment.
Excellent written and verbal communication.
Must hold a valid driver's license, have access to an operational vehicle and be able to commute and travel for work.
Strongly prefer a candidate that will have a demonstrated understanding of and competence in serving culturally diverse populations.
