Collect data for submission of DPH quarterly report
1. Leads in the continuity of care for patients on the unit in relation to the BSAS regulations and guidelines for treatment and also in compliance with the managed care based expectations of treatment. Drives a culture of sobriety and recovery for treatment team, staff and patients.
2. Maintain the documentation necessary in the EMR per department and hospital requirements.
3. Maintains current knowledge of patient's legal status and legal issues ensures appropriate documents are available in chart and filed in a timely manner.
Martha’s Vineyard Community Services new Recovery Support Center is seeking a Program Director to provide general oversight of all services, budgeting, reporting and programming. This position will be responsible for program implementation, sustainability and maintaining a safe, welcoming space for all members. Lived experience in recovery from substance addiction or co-occurring disorder preferred with comprehensive understanding of substance use disorders, addiction, and recovery. Demonstrated knowledge and competency on substance addiction and recovery will be considered.
The Social Work Care Manager is a key member of our primary care based complex care management teams of nurses, social workers and community health workers. Care managers work with high risk/high cost patients and health care teams to achieve better health, cost, and quality of life outcomes. The Skills Instructor will provide the following to assigned individuals: developmental skills training, implementing therapeutic service plan (DHSP) for all assigned individuals and complete documentation which supports such plan.

Responding to the growing need for access to ambulatory substance use disorder services (SUDs), Brigham Health is opening a Bridge Clinic, which will be an outpatient clinic dedicated to provide intensive ambulatory treatment for patients with SUDs (including medication-assisted treatment and individual and group support) and connect them to long-term, community based treatment and resources.
Reports to the wellness manager. Responsible for the implementation and maintenance of an effective trainee employee assistance program (TEAP) with emphasis placed on awareness, prevention, staff training and networking with community resources, in compliance with government and management directives.
1. Assist in developing procedures to coordinate and integrate TEAP components with other center programs, policies and zero tolerance.
