Drug Alcohol Clinical Care Coordinator

1. Leads in the continuity of care for patients on the unit in relation to the BSAS regulations and guidelines for treatment and also in compliance with the managed care based expectations of treatment. Drives a culture of sobriety and recovery for treatment team, staff and patients.
2. Maintain the documentation necessary in the EMR per department and hospital requirements.
3. Maintains current knowledge of patient's legal status and legal issues ensures appropriate documents are available in chart and filed in a timely manner.

Responding to the growing need for access to ambulatory substance use disorder services (SUDs), Brigham Health is opening a Bridge Clinic, which will be an outpatient clinic dedicated to provide intensive ambulatory treatment for patients with SUDs (including medication-assisted treatment and individual and group support) and connect them to long-term, community based treatment and resources.

Recovery Coach

The Recovery Coach will support the recoveree in their recovery process, linking recoverees to the recovery community and serving as a personal guide and mentor. Will develop recovery wellness plans and utilize personal experience to help recoveree overcome obstacles to recovery. Must have reliable transportation and either a Bachelor’s degree or 3 yrs lived experience (personal or friends and family) of substance use.
